Qualified Intellectual Disabilities Professional (QIDP)/ Program Manager:
(Reports to the Executive Director )
Seeking applicant for part-time position with our Group home for adults with Intellectual disabilities. Coordinate Individual Service Plans (ISP) every year to assess individual’s progress, insure participation in valued activities of individual’s choice and establish training programs for daily living skills. Insure completion of all paperwork in a timely manner including but not limited to quarterly reviews, ISP’s, assessments, data forms, various medication sheets, behavior charts, and quarterly QA reports. Coordinate with other agencies and departments, as well as parents and advocates on individuals behalf to solve problems and provide assistance. Attend the individual’s ISP meetings or designate appropriate QIDP staff to attend individuals ISP meetings two times per year. Maintains day to day operation of the program. Conducts and/or reviews performance evaluations of staff in accordance with policy. Monitors staff performance on regular basis and provides feedback. Meets regularly with the community, monitors the documentation process, orientate, trains and supervise the implementers. Prepares staffing reports, develop and ensures the implementation of individual assessments. Administer medication. Participates in Interdisciplinary Team Staffing, make program modifications as needed. Coordinate all services offered to each individual. Participate in discharge planning, monitor’s behavioral management. Ensures individuals exhibit signs of good personal hygiene; investigates individual rights.
Knowledge, Skills and Abilities:
You MUST have (no exceptions will be made)

at least five (5) years of PAID experience coordinating Individual Service Plans (ISP), if you do not have a degree

or a Bachelor’s degree in human services field including social work, psychology, counseling, special
Computer skills and receptionist duties. Risk management operations, Financial record-keeping and spending, timekeeping and payroll, Familiarity with audit procedures, Knowledge of behavioral interventions, medication administration. At least (5) years of supervisory and professional experience working with adult individuals that have a diagnosed of intellectual disabilities.

Direct Support Professional Job Description:
(Reports to the Program Manager)
DSP will serve as direct care worker having direct responsibility and supervision of the individual’s to whom he/she is assigned. Oversee physical care, development of acceptable habits and attitudes; management of individual behavior and helping to meet the goals and objectives of any service plan. Participate in individual day support assessments; ensuring the daily needs of the individuals is properly met. Escorts individuals to and from training and community activities as assigned. Provides direct supervision and guidance to ensure safety and welfare of individuals. Conduct and document monthly fire drills according to DMR procedures. Teach individuals basic First-Aid, fire safety, emergency skills. Observe individuals daily for behavioral changes, physical injuries/conditions and cleanliness, administer medication, and conduct accordance with Individual Service Plan. Performs required housekeeping duties in order to maintain a clean, safe and orderly environment.
Knowledge, Skills and Abilities
High School Diploma or equivalent (GED) and preferably a minimum of one year of work experience in either a residential program or working with individuals that have intellectual disabilities. Good oral and written communication skills.
